Utilisation of brain MRI for the diagnosis of dementia in low and middle-income countries: systematic review and meta-analysis

Objectives

To synthesise the prevalence and patterns of dementia-relevant structural brain MRI abnormalities in adults with suspected or confirmed dementia in low- and middle-income countries (LMICs), and to summarise MRI protocols and the incremental diagnostic contribution of MRI beyond cognitive screening.

Design

Systematic review and meta-analysis.

Data sources

PubMed, EMBASE, Web of Science and PsycINFO (January 1990–27 January 2025), plus reference list screening and targeted manual searches.

Eligibility criteria

Observational or diagnostic-accuracy studies from World Bank-defined LMICs including adults (≥50 years) with suspected or confirmed dementia who underwent brain MRI as part of diagnostic evaluation.

Data extraction and synthesis

Two reviewers independently screened, extracted data and assessed risk of bias using ROBINS-I. Random-effects models pooled prevalence of dementia-relevant MRI abnormalities; diagnostic-accuracy outcomes were synthesised narratively due to heterogeneous reference standards and incomplete reporting.

Results

39 LMIC studies were included; 23 studies (2513 participants) contributed to the meta-analysis. Dementia-relevant MRI abnormalities (defined as ≥1 clinically relevant structural abnormality per study definition) were present in 1248/2513 participants. The pooled prevalence of dementia-relevant MRI abnormalities was 58% (95% CI 43% to 72%), with substantial heterogeneity (I²=95%) and a wide prediction interval (8–96%), indicating marked between-study variability; this estimate should be interpreted as a descriptive summary of study-level proportions rather than a precise population parameter.

Conclusions

Brain MRI frequently demonstrates dementia-relevant pathology in LMIC clinical cohorts, usually with mixed neurodegenerative-vascular patterns. Structured visual ratings may add aetiologic specificity beyond cognitive screening, but pooled estimates should be interpreted as summaries of heterogeneous study-level findings rather than precise population parameters, given high heterogeneity and risk of bias.

Identification of core genes mediating the association between obesity and hepatocellular carcinoma: A bioinformatics study based on mitochondrial metabolism and immune pathways

Purpose

Obesity is strongly associated with hepatocellular carcinoma (HCC), yet the molecular mechanisms linking them remain unclear. This study aimed to identify mitochondrial metabolism-related genes bridging obesity and HCC and to investigate their role in regulating the metabolic-immune microenvironment.

Methods

Public transcriptomic datasets from obesity (derived from peripheral blood mononuclear cells) and HCC (derived from liver tissue) cohorts were integrated. A multi-step bioinformatic pipeline combining differential expression analysis (DEA), weighted gene co-expression network analysis (WGCNA), and machine learning (ML) algorithms was applied to identify and validate hub genes. Associations with the tumor immune microenvironment were assessed using ssGSEA and correlation analyses.

Results

27 core genes were identified, significantly enriched in lipid metabolism and immune response pathways. Among these, ML highlighted ACAA1 and ADI1 as downregulated candidate genes. While discovery datasets showed high diagnostic potential, ADI1 exhibited more variable performance in obesity external validation compared to the robust consistency of ACAA1. Downregulation of both genes correlated with effector T/NK cell lipid-mediated functional exhaustion and disrupted networks of immune checkpoints and chemokines, reflecting an immunosuppressive microenvironment.

Conclusions

ACAA1 and potentially ADI1 are downregulated candidate genes linking obesity to HCC. Their suppression likely drives obesity-related HCC progression by coupling mitochondrial metabolic reprogramming with immunosuppressive tumor microenvironment remodeling, representing potential therapeutic targets.

Introduction

The utility of brain MRI in dementia diagnosis offers critical insights into structural brain changes, such as hippocampal and thalamic atrophy, which are hallmark features of Alzheimer’s disease and Alzheimer’s disease-related dementias . However, its use, especially in low- and middle-income countries (LMICs), is affected by limited accessibility. This protocol outlines a systematic review and meta-analysis to assess the diagnostic utility, feasibility and challenges of integrating brain MRI for dementia diagnosis in LMICs.

Methods and analysis

The review follows Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols with a priori eligibility criteria and is registered in PROSPERO. Searches (from inception to September 2025) will be run in MEDLINE/PubMed, EMBASE, Web of Science and PsycINFO, with supplementary bibliography screening. Adults ≥50 years in LMIC settings undergoing brain MRI for dementia evaluation will be eligible. Data will be synthesised narratively and, where appropriate, via random-effects meta-analysis with planned subgroup analyses by MRI approach (qualitative vs quantitative), magnet strength, sequence availability and reference standard. Screening and data extraction are planned for 1 November to 30 December 2025.

Progression from uncomplicated to severe malaria among children in settings receiving different malaria control interventions in sub-Saharan Africa: a systematic review protocol

Background

Different malaria control measures are deployed simultaneously in endemic settings globally, with varying impacts on malaria burden. In sub-Saharan Africa, which bears the greatest burden of malaria, evidence on the impact of implementing various control interventions on malaria immunity remains unknown. This systematic review seeks to collate evidence on the extent of progression from uncomplicated to severe malaria among populations in sub-Saharan Africa settings receiving concurrent deployment of various malaria control measures.

Methods

The review will use a priori criteria contained in the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols. An experienced librarian (AAK) will independently search for articles from the following databases: PubMed, Web of Science, Embase, Scopus and Google Scholar. Boolean operators ‘AND’ and ‘OR’ will be used in the article search. Identified articles will be managed using EndNote. Article screening for inclusion and data extraction will be done in duplicate by two reviewers (EJO, and BM). Data extraction tools will be developed and customised in Excel. Data will be analysed using both narrative and quantitative synthesis. The level of heterogeneity between study outcomes will be measured using the I2 statistic. Subgroup analysis will be conducted to explore heterogeneity and establish the impact of different control interventions on progression from uncomplicated to severe malaria. A full systematic review and meta-analysis is expected to be ready for dissemination by the end of December 2025.

Ethical consideration and dissemination of findings

This study did not involve human participants and so ethical approval was not sought. A full review and a meta-analysis will be published in a peer-reviewed journal and presented at national and international conferences.

Desempeño de los profesionales de enfermería

El presente artículo tiene como objetivo describir las experiencias de los profesionales de enfermería en su desempeño laboral. Para tal fin se planteó un estudio de enfoque cualitativo, fenomenológico e interpretativo. Este abordó la realidad construida por los individuos en su ambiente socio – laboral, por lo tanto, la verdad emerge de los diversos significados que las personas dan a las situaciones que viven. La muestra fue intencionada, se aplicó a enfermeros(as) egresados de diferentes programas académicos, el método de recolección fue una entrevista virtual, previo diligenciamiento y firma de consentimiento informado. Del análisis de datos emergen las siguientes categorías: reconocimiento social de la profesión, dedicación de tiempo, atributo de la profesión, condición laboral, ética del cuidado, saber disciplinar, significado de la formación, expectativa laboral, miedo a lo desconocido, estatus profesional, ámbito de desempeño y motivación y compromiso. Esta investigación tuvo en cuenta las consideraciones éticas para la Investigación en salud. Como conclusión, los participantes del estudio se sienten satisfechos con su profesión, a pesar de las condiciones adversas y la situación de pandemia por la cual atraviesa el mundo, consideran ser parte fundamental y aportar su conocimientos y habilidades para trasformar la sociedad.
