This study aimed to evaluate the effects of rumen-protected methionine (RPM) supplementation on productive and physiological responses of primiparous Holstein cows during summer. We hypothesized that RPM supplementation would maintain or improve milk yield and composition due to beneficial physiological, redox, and inflammatory responses in cows exposed to summer heat. The trial was conducted in a randomized block design during nine weeks in Brazil using 80 primiparous cows (182 ± 64 DIM; 42.9 ± 4.7 kg/d milk). Cows were blocked by milk yield and DIM and assigned to a control diet (CON; no added RPM) or the same diet supplemented with RPM (Mepron®, Evonik) at 0.75 g/kg diet dry matter, targeting 20 g/cow/day (product contains 62% metabolizable methionine) to the average cow. Milk yield and composition, vaginal temperature, respiratory rate, and plasma samples were collected in weeks 3, 6, and 9. Data were analyzed using mixed models including treatment, week, and their interaction as fixed effects, and block and cow as random effects. Cows were maintained under naturally occurring summer conditions. Environmental monitoring during weeks 3, 6, and 9 indicated elevated temperature–humidity index (THI) values, with values remaining above the heat-stress threshold (THI > 68) for 68.3% of the monitored hours (mean THI = 70.6; range 61.0–84.4). Overall (least squares mean across weeks 3, 6, and 9), RPM increased milk yield by 2.0 kg/d (44.9 vs. 42.9 kg/d), protein yield by 50 g/d (1,464 vs. 1,414 g/d), lactose yield by 108 g/d (2,109 vs. 2,001 g/d), and total solids yield by 176 g/d (5,331 vs. 5,155 g/d). Lactose concentration was lower in RPM (4.71 vs. 4.76%). Fat yield was unaffected, but a treatment × week interaction was observed for fat content. Milk fatty acid (FA) profile was unchanged, although treatment × week interactions were observed for individual fatty acids (C16:0, C18:0, C18:1, and preformed FA). Plasma glucose was lower, and insulin was higher in RPM than in CON cows (39.3 vs. 43.2 mg/dL and 0.52 vs. 0.35 ng/mL, respectively). Antioxidant capacity improved, with RPM cows having greater ferric reducing antioxidant power (32.9 vs. 28.5 µM) and lower malondialdehyde (2.48 vs. 2.78 nmol/mL). Other biochemical, inflammatory, and immune markers were unaffected. Respiratory rate was slightly higher in RPM than in CON cows (55 vs. 50 breaths/min). Mean vaginal temperature did not differ between treatments; however, a treatment × time × hour interaction was observed. Supplementation with RPM improved milk and solids yield, and enhanced antioxidant capacity and insulin levels, supporting its use to improve metabolic resilience under warm conditions.Current research lacks a comprehensive understanding of evidence-based practice (EBP) adoption and its predictors across diverse healthcare professionals (HCPs) in the Eastern Mediterranean Region (EMR), particularly with a direct comparison between nurses and other professional groups.
This study aims to evaluate the EBP competencies, adoption levels, identified barriers, and associated predictors among nurses and other healthcare professionals (HCPs) within the EMR.
A cross-sectional, correlational, and comparative design was used. An electronic survey was distributed (April 27th–August 17th, 2023) via convenience/snowball sampling, inviting nurses, physicians, physiotherapists, dentists, and pharmacists across the EMR to complete the survey. Multivariate regression analysis and structural equation modeling (SEM) were used to identify predictors of EBP adoption.
A total of 4673 HCPs participated and reported several barriers to EBP adoption, including time constraints, difficulties in interpreting statistics, lack of authority to change practices, and insufficient equipment. Nurses had fewer postgraduate degrees but more work experience and full-time employment than other HCPs. Despite reporting more workplace EBP support, nurses read less research, had lower EBP scores and adoption propensity, and perceived greater barriers than other HCPs (p < 0.002). Multivariate regression showed the highest barrier scores in Syria/Tunisia and the lowest in the United Arab Emirates. Nurses reported significantly higher barriers compared to other HCPs (p < 0.001). Fear of Change was not a primary barrier overall, but was elevated in specific subgroups of nurses. SEM showed good fit: RMSEA = 0.077, SRMR = 0.053, CFI = 0.80, χ 2(df) = 917, p < 0.001. SEM showed that EBP adoption propensity and fear of change significantly mediate the relationship between HCPs' characteristics and EBP scores.
Despite having a positive attitude and propensity towards EBP, nurses lack the necessary knowledge and support to adopt it, and they face more barriers than other healthcare professionals. Nurses require greater support from healthcare leaders to enhance their EBP competencies and address the reported barriers. Policymakers and organizations should prioritize tailored, role-specific training and supportive structures and environments to ensure equitable and effective EBP implementation for improving patient outcomes across the EMR.

Iodine-based antiseptics are essential in wound care but are often limited by cytotoxicity, instability, and rapid iodine release. Novostron is a novel polymer–iodine complex incorporating dextrin, polyvinyl alcohol, and metal ions, designed to enable controlled iodine release. Structural integrity and composition were confirmed by ¹H and 13C NMR spectroscopy and physicochemical analysis, indicating a molecular weight of ~9500 g/mol, a pH of 4.23, and an iodine content of 8.13%. Pharmacokinetic analysis in rabbits demonstrated that following a single dermal application, systemic iodine absorption was minimal, with peak blood iodine concentrations remaining within physiological limits and rapid elimination within 24 hours. Evaluation of thyroid function revealed no significant changes in serum T₃, T₄, or TSH levels compared with those of the controls, confirming that topical application of Novostron does not disrupt thyroid homeostasis. In compliance with OECD guidelines in rabbits, guinea pigs, and rats, Novostron showed no signs of dermal irritation, skin sensitization, or systemic toxicity (LD₅₀ > 2000 mg/kg). In a rat cotton pellet granuloma model, Novostron significantly reduced the inflammatory mass (23.65% inhibition), supporting its anti-inflammatory potential. In a murine burn model, Novostron accelerated wound contraction (25.95% at day 10), increased epidermal thickness, and enhanced collagen deposition (~44%), outperforming controls and matching or exceeding betadine. These findings suggest that Novostron promotes tissue repair by modulating inflammation. Overall, Novostron demonstrated a favourable preclinical safety and efficacy profile, and its polymer–iodine composition, which enables controlled release and localized activity highlights its potential as a promising topical therapeutic. However, the study was limited to animal models and short-term observation; further long-term and clinical investigations are needed to confirm its translational potential in human wound healing.This study investigates how environmental and household variables relate to groundwater quality in Indonesia.

Out of 5965 households, only 5.15% had emptied faecal sludge (FS) in the last 3 years, while E. coli contamination was detected in 71.21% of the samples. Improved drinking water sources and proper FS emptying practices were associated with better water quality.
These findings highlight the urgent need for enhanced surveillance and evidence-based policymaking to safeguard both drinking water quality and sanitation practices, which are crucial for public health and environmental sustainability.
Research priorities guide research activities, funding and resources within health services. To ensure that research efforts are meaningful and impactful, it is vital that organisational research agendas reflect the priorities of both healthcare consumers and staff, alongside broader national and international research frameworks. This paper outlines a research priority-setting project conducted across two hospitals in Western Australia, aimed at identifying shared research priorities through a collaborative and inclusive approach.
To identify the top ten nursing and allied health research priorities for two hospitals in Western Australia.
A modified James Lind Alliance Priority Setting Partnership approach was used, involving health services users, nurses, allied health professionals, and community members in a co-design approach across three phases. In phase 1, four community conversations were conducted to elicit an initial set of research topics. This data-informed phase 2, a survey to collect diverse views from a wider participant pool. In phase 3, a pre-selected sample of potential research priorities was discussed in a consensus workshop to reach a group consensus of the top ten research priorities. Qualitative data was analysed using multi-step thematic analysis, and quantitative data was analysed using descriptive statistics.
A total of 67, 151 and 18 people participated across study phases 1, 2 and 3, respectively, comprising nurses, allied health professionals, healthcare users, carers, and interested community members. The top ten research priorities reflected three areas: healthcare systems re/design (streamlining care; access to healthcare; patient journey and quality of care), workforce needs (workforce well-being, retention and adequate staffing; workforce training), and specific health issues and needs (dementia and delirium; mental health; caring for carers; Aboriginal and Torres Strait Islander health; palliative care and elderly people).
The research priorities identified in this study for two hospitals in Western Australia reflect the strong desire of nurses, allied health professionals, healthcare users and community members to improve structural issues in healthcare systems. This includes how healthcare systems are designed and integrated with each other, how workforce needs affect service delivery, and a greater focus on holistic service provision for specific health issues and needs.
Healthcare consumers were an integral part of this study. Healthcare consumers were involved in the design of the study, the conduct of the study, and the review of the data analysis.
Objetivo principal: Analizar el papel de la enfermera pediátrica en el abordaje del neonato con gastrosquisis en un contexto de consumo de opioides durante el embarazo. Presentación del caso: Neonato con gastrosquisis, en cuyos antecedentes destaca el consumo de marihuana, por parte de la madre, durante el embarazo. Valoración: Según los 11 patrones funcionales de M. Gordon, se observó una alteración en los patrones 1, 2, 3, 5 y 7. Plan de cuidados: Destaca la identificación los diagnósticos NANDA Deterioro de la integridad tisular, Riesgo de motilidad gastrointestinal disfuncional y Síndrome de abstinencia neonatal. Se planificaron los criterios de resultado e intervenciones pertinentes, observándose una mejoría sustancial del paciente a las tres semanas. Discusión: El papel de la enfermera pediátrica en casos de este tipo, se basa en el cuidado integral del neonato, con intervenciones dirigidas, especialmente, al mantenimiento del confort y del estado hemodinámico, respiratorio y gastrointestinal del neonato.
Objetivo: evaluar la calidad de vida de los cuidadores de niños con necesidades especiales de salud. Método: estudio cuantitativo, en una Unidad de Pediatría, con 16 madres de niños con necesidades especiales de salud. La recolección de datos se produjo mediante la aplicación del instrumento WHOQOL-bref. Para el análisis de los datos se utilizó la estadística descriptiva e inferencial. Resultados: la calidad de vida de los familiares cuidadores es influenciada por los aspectos físicos; psicológica; del medio ambiente y de las relaciones sociales. Conclusiones: la percepción general de la calidad de vida de estos cuidadores no puede considerarse satisfactoria, ya que los valores asociados a todos los ámbitos son relativamente bajos.
Objetivo principal: determinar si se puede establecer la lactancia materna en mujeres que se hayan realizado cirugía de aumento o reducción mamaria. Metodología: se realizó una revisión bibliográfica y se propusieron como criterios de inclusión que los documentos utilizados estuviesen publicados en español, inglés o portugués y en los últimos 10 años. El criterio de exclusión era descartar estudios realizados en animales. Resultados: se obtuvieron 8 artículos para revisar: 3 eran estudios observacionales y 5 revisiones bibliográficas. En 7 de los 8 artículos se concluyó que establecer la lactancia materna es posible tras una mamoplastia. Conclusión principal: es posible establecer la lactancia materna tras una cirugía de reducción o aumento mamario, aunque puede existir mayor dificultad que en la población general. Por ello, es necesario que los profesionales sanitarios acompañen y ayuden a la mujer durante la lactancia materna.
Este estudio centra su objetivo principal en describir las vivencias de una madre ante el nacimiento de un hijo prematuro; las repercusio-nes sobre el rol materno y paterno, y la conducta referida de su hijo mayor. Para ello se ha realizado una entrevista en profundidad a la madre sobre sus propias vivencias y reflexiones, pasados tres años desde el evento. Se describen las categorías extraídas de su relato, observando cuestiones relacionadas con sus sentimientos ante el embarazo y el parto prematuro, el rol materno, el apoyo familiar perci-bido, y las experiencias del marido y del hermano mayor desde la perspectiva de la madre. El apoyo de enfermería durante la hospitali-zación del niño prematuro sugiere un abordaje integral, familiar y humanizado para minimizar el impacto del nacimiento prematuro y propiciar un establecimiento adecuado del rol paterno en todos los hijos de la pareja.
Objetivo: analizar los componentes de la Red de Atención Psicosocial (RAPS) orientadas a la atención de adolescentes usuarios de crack. Método: estudio exploratorio y descriptivo con enfoque cualitativo de los datos, desarrollado en el Centro de Atención Psicosocial Alcohol y Drogas (CAPSad) y en el Centro de Atención Psicosocial Infanto-juvenil (CAPSi). Participaron 20 profesionales del CAPSad y 10 del CAPSi. Los datos se recopilaron mediante entrevistas semiestructuradas y se analizaron conforme al Análisis Temático. Resultados: se identificaron como componentes de la RAPS orientadas a la atención de los adolescentes usuarios de crack: Atención Psicosocial Especializada; Atención Residencial de Carácter Transitorio; Atención Hospitalaria – Hospital Psiquiátrico y Atención Primaria – Unidad de Atención Primaria. Conclusión: el desempeño del CAPSad se subrayó en la atención de los adolescentes usuarios de crack, con acciones dirigidas a rescatar los lazos escolares y en entornos más saludables, así como la atención a la familia de estos jóvenes.
Objetivo: Analizar el papel del enfermero en el abordaje inicial del recién nacido afecto de síndrome de bridas amnióticas. Presentación del caso: Recién nacido prematuro con lesión por banda amniótica en muñeca derecha. Valoración: Según los 11 patrones funcionales de Gordon, se objetivó una alteración los patrones 2 y 6 en el neonato, y del 7 en los padres. Plan de Cuidados: Se identificaron los diagnósticos de Riesgo de perfusión tisular periférica ineficaz, Riesgo de deterioro de la integridad tisular, Dolor agudo y Ansiedad. Tras planificar los criterios de resultado y ejecutar las intervenciones pertinentes, se observó la no aparición de los riesgos identificados, resolución del dolor y disminución de la ansiedad de los padres. Discusión: El papel del enfermero en casos de este tipo, se basa en la inspección de la lesión para minimizar la aparición de complicaciones, el control del dolor y el apoyo emocional a la familia.
