This work aimed to clarify how polymorphisms in the TNF gene relate to metabolic syndrome (MetS), type 2 diabetes mellitus (T2DM), and a broad spectrum of cardiometabolic characteristics, while also determining their impact on circulating TNF‑α concentrations. A total of 765 participants were genotyped for rs1800629 and rs361525, and serum TNF-α was also measured. To assess these relationships, multivariable logistic regression models—incorporating age, sex, and body mass index (BMI)—were applied to estimate adjusted odds ratios (aORs) and their corresponding 95% confidence intervals (CIs). Both variants were significantly associated with MetS: rs1800629 (crude OR = 2.22, 95% CI: 1.45–3.44, P P P = 0.035; adjusted OR = 2.84, 95% CI: 1.17–7.31, P = 0.025). The rs1800629 variant was also linked to T2DM risk (adjusted OR = 2.61, 95% CI: 1.35–5.24, P = 0.006), whereas rs361525 showed no such association. Carriers of rs1800629 had higher mean TNF-α levels (PSocioeconomic inequalities significantly impact access to healthcare services for individuals with type 2 diabetes mellitus (T2DM). This study investigates these inequalities in Iran, focusing on factors such as asset, sex, urban–rural residence, age, education, employment status, and marital status.
Cross-sectional study.
This study used data from the national ‘Diabetes Care (DiaCare)’ study, a population-based survey conducted from 2018 to 2020 in Iran.
Socioeconomic status (SES) was assessed using Principal Component Analysis (PCA) based on assets. Socioeconomic inequalities in access to physicians, pharmacies and laboratories were measured using the Concentration Index (CI) and Erreygers Corrected Concentration Index (ECI). Decomposition analysis was performed using a probit regression model to assess the contributions of various factors to the observed inequalities.
Among 13 315 patients with T2DM, 5.8% lacked access to physicians, 6.8% to pharmacies and 8.7% to laboratories. The CI was positive and statistically significant for access to physicians (0.0614), pharmacies (0.0787) and laboratories (0.0875), indicating better access concentrated among higher SES individuals. Urban residents had the largest positive marginal effects on access to physicians (0.032), pharmacies (0.078) and laboratories (0.053), with percentage contributions of 13.21%, 23.23% and 17.39%, respectively. Higher asset quintiles showed substantial contributions to inequalities, with the highest quintile contributing 10.5% to physician access inequality, 9.68% to pharmacy access and 9.16% to laboratory access. Education level also positively impacted access, with high school education contributing 0.64% and college education 0.52% to access inequalities. Sex differences showed a negative marginal effect for women, indicating slightly lower access.
Socioeconomic factors, particularly asset, residence and education, significantly impact access to healthcare services for patients with T2DM in Iran. Policies should focus on reducing barriers to healthcare access, especially for lower SES and rural populations.
To examine the associations between the inflammatory potentials of diet and lifestyle, as measured by the Dietary Inflammation Score (DIS) and Lifestyle Inflammation Score (LIS), with the risk of type 2 diabetes mellitus (T2DM), hypertension and chronic kidney disease (CKD).
This retrospective cohort study used data collected between 2000 and 2012 from the Multi-Ethnic Study of Atherosclerosis cohort, which was conducted across six US communities.
The study sample included 4736 participants for the analysis of T2DM, 2149 participants for hypertension and 4631 participants for CKD.
The primary outcomes were the incidence of T2DM, hypertension and CKD during follow-up.
During a median follow-up of 9.0 years, 537 participants developed T2DM, 1019 developed hypertension and 1067 developed CKD. DIS was not associated with T2DM in the overall population; however, women in the third quartile of DIS had a 52% higher risk of developing T2DM (HR 1.52; 95% CI 1.07 to 2.15). Also, individuals in the third quartile of DIS had a 22% higher risk of hypertension (1.22; 95% CI 1.02 to 1.47) in the overall population; however, no significant linear trend was observed across the quartiles (P-trend=0.397). This study highlights the role of diet and lifestyle-related inflammation in the development of hypertension and T2DM risk, providing novel evidence from a large, multiethnic US cohort. The findings underscore the potential of dietary and lifestyle strategies that target inflammation to reduce cardiometabolic disease risk.

PurposeLarge language models (LLMs) have the potential to change medical education. Whether LLMs can generate multiple-choice questions (MCQs) that are of similar quality to those created by humans is unclear. This investigation assessed the quality of MCQs generated by LLMs compared to humans.
MethodsThis review was registered with PROSPERO (CRD42025608775). A systematic review and frequentist random-effects network meta-analysis (NMA) or pairwise meta-analysis was performed. Ovid MEDLINE, Ovid EMBASE, and Scopus were searched from inception to November 1, 2024. The quality of MCQs was assessed with seven pre-defined outcomes: question relevance, clarity, accuracy/correctness; distractor quality; item difficulty analysis; and item discrimination analysis (point biserial correlation and item discrimination index). Continuous data were transformed to a 10-point scale to facilitate statistical analysis and reported as mean differences (MD). The MERSQI and the Grade of Recommendations, Assessment, Development and Evaluation (GRADE) NMA guidelines were used to assess risk of bias and certainty of evidence assessments.
ResultsFive LLMs were included. NMA demonstrated that ChatGPT 4 generated similar quality MCQs to humans with regards to question relevance (MD −0.13; 95% CI: −0.44,0.18; GRADE: VERY LOW), question clarity (MD −0.03; 95% CI: −0.15,0.10; GRADE: VERY LOW), and distractor quality (MD −0.10; 95% CI: −0.24,0.04; GRADE: VERY LOW); however, MCQs generated by Llama 2 performed worse than humans with regards to question clarity (MD −1.21; 95% CI: −1.60,-0.82; GRADE: VERY LOW) and distractor quality (MD −1.50; 95% CI: −2.03,-0.97; GRADE: VERY LOW). Non-smoking-related lung cancer is increasingly associated with environmental factors such as particulate matter (PM) exposure. Using deep small RNA sequencing, we identified distinct miRNA expression patterns in lung cancer patients compared to non-cancer controls, stratified by smoking status. Notably, hsa-miR-125b-5p and hsa-miR-100-5p were significantly downregulated in non-smoking lung cancer patients. Pathway enrichment analysis revealed smoking amplifies pathways related to glycan biosynthesis, signal transduction, and transcriptional regulation, while non-smoking lung cancer is characterized by immune dysfunction and metabolic alterations, including oxidative phosphorylation and natural killer cell cytotoxicity. Validation in a larger cohort using quantitative RT-PCR confirmed the suppression of miR-125b-5p and miR-100-5p in non-smoking lung cancer patients. Additionally, miR-203a and miR-199a-3p were identified as potential biomarkers for lung cancer, independent of smoking status. Chronic PM exposure in primary bronchial/tracheal epithelial cells initially elevated miR-125b-5p and miR-100-5p expression, but prolonged exposure suppressed these miRNAs while increasing their target genes, TXNRD1 and HOXA1, suggesting stress-induced dysregulation. Functional studies using miRNA mimics demonstrated that miR-125b-5p and miR-100-5p suppress PM-induced cancer cell mobility and colony formation, with miR-125b-5p exhibiting broader effects. These findings underscore the critical roles of miR-125b-5p and miR-100-5p in PM-associated lung cancer progression and their potential as biomarkers and therapeutic targets. BackgroundUpper extremity (UE) dominance is often defined by self-reported hand preference; however, this may not accurately reflect true functional or strength-based dominance. This study examined the relationship between writing hand, throwing hand, and maximal grip strength to assess how these measures align.
MethodsThirty-four healthy, recreationally active college-aged individuals reported their preferred writing and throwing hands and completed standardized grip strength testing. Associations among the variables were analyzed using Phi coefficients and chi-square tests.
ResultsA moderate, significant relationship was found between writing and throwing hand preference (φ = 0.456; p = 0.008), indicating general consistency across these subjective measures. However, no significant association emerged between grip strength dominance and either writing (φ = 0.027; p = 0.876) or throwing hand (φ = 0.096; p = 0.574).
ConclusionThese results suggest that grip strength dominance may not correlate with commonly used indicators of hand preference, highlighting the need for task-specific definitions of dominance in clinical and athletic contexts. Consequently, employing such task-specific definitions allows for more accurate assessments and enhances the translational relevance of research findings in practical settings.
Patient and family engagement in patient safety in low- and lower-middle-income countries (LLMICs) is not well described. This scoping review synthesised the literature to identify key strategies, barriers and enablers of engagement and to explain how patients and families contribute to safety efforts in healthcare settings.

PubMed, Medline, CINAHL, PsycINFO, Scopus and Web of Science were searched from inception to April 2024.
We included qualitative, quantitative and mixed-methods studies, published in English across all healthcare settings, that met the following criteria: (1) explored the attitudes, perceptions and experiences of patients, families or healthcare providers (HCPs) regarding patient engagement in safety activities; (2) described the participation of patients, caregivers or families in the design, delivery or evaluation of interventions aimed at promoting patient safety in healthcare facilities.
Data were extracted using a standardised charting form, and a narrative synthe.sis was conducted to identify key themes. The National Health Service (NHS) framework for patient engagement in patient safety was applied to categorise forms of engagement (information, involvement and partnership) and levels of engagement (own care, service provider and system) among included studies.
Of 7709 records screened, 36 studies met inclusion criteria. Twenty-three studies explored the perspectives of patients, families and HCPs on patient engagement in safety. Key findings revealed that while patients and families recognised the importance of their roles in safety, unclear role definitions, lack of training and communication barriers limited their participation. HCPs acknowledged the value of patient involvement but identified challenges such as knowledge gaps, time constraints and lack of institutional support. Thirteen studies involved patients and family caregivers in safety improvement initiatives. Most interventions were limited to information sharing, with only two achieving partnership-level engagement. Engagement primarily occurred at the individual and provider levels, with no studies addressing the policy level. Factors influencing engagement were identified at four levels: patient, HCP, system and community.
Despite its potential benefits, the implementation and impact of patient and family engagement in patient safety efforts in LLMICs remain largely unexplored. Most efforts focus on information sharing through educational interventions at the direct care level, with limited involvement of patients and families at the partnership level. To enhance safety outcomes, future initiatives must adopt context-specific strategies that empower patients and families as active partners. Drawing lessons from high-income countries and adapting them to the socio-cultural and infrastructural realities of LLMICs will be crucial for developing scalable, cost-effective and sustainable engagement practices.
The present study aimed to develop and psychometrically validate a culturally sensitive instrument, the women’s empowerment in dealing with intimate partner violence (WED-IPV) scale, in Iran.
A mixed-methods research study to collect and analyse quantitative and qualitative data.
Social emergency centres, healthcare centres and psychotherapy clinics in three Iranian metropolises.
The participants were 31 married women in the qualitative phase and 420 in the quantitative phase.
The primary outcome was the validation of the WED-IPV scale, including the assessment of face, content, construct validity and the assessment of internal consistency through Cronbach’s alpha, test–retest reliability through intraclass correlation coefficients (ICC) and stability of the factor structure. Secondary outcomes included the assessment of WED-IPV indicators in women using values obtained from levels specified according to the Interval of SD from the Mean formula for each indicator and total empowerment.
The scale development process refined the initial 72 items down to 28 key items through expert review and validity testing. Exploratory factor analysis identified five factors: self-esteem, attitude, self-efficacy, access to support resources and economic empowerment that explained 65.72% of the variance. The scale showed excellent reliability (Cronbach’s α=0.97; ICC=0.95) and valid associations with related measures. Analysis of completed questionnaires of 412 women revealed that a significant portion scored low or moderate in each domain: 41% in self-esteem, 35% in self-efficacy, 39% in attitude, 47% in access to support and 45% in economic empowerment.
The WED-IPV scale is a psychometrically robust, reliable and valid instrument for assessing women’s empowerment concerning IPV in Iranian contexts. Its brevity and user-friendly design facilitate routine application in primary and secondary care settings. Future research should explore cross-cultural validation and predictive validity in longitudinal studies to inform targeted interventions and policy formulation.
Cancer, as a significant global health challenge, poses numerous difficulties for those affected. Self-advocacy, which refers to an individual’s capacity and desire to articulate their preferences and needs when confronted with disease-related obstacles, is crucial in enhancing their quality of life. Given the absence of a validated instrument for assessing self-advocacy in the Iranian patient population with cancer, this protocol aims to describe the psychometric properties of the Self-Advocacy in Cancer Survivorship (SACS) Scale in Persian among Iranian patients with cancer.
In this methodological study, the SACS Scale will be translated into Persian following the WHO guidelines. Subsequently, the scale’s validity, including face validity, content validity, construct validity (confirmatory factor analysis, exploratory factor analysis and convergent validity) and reliability (internal consistency and test–retest reliability), will be evaluated using SPSS V.24 and Amos V.24 software, along with appropriate statistical tests. Study participants will be patients with cancer in Ardabil, selected through convenience sampling.
This study, forming part of a master’s thesis, received ethical approval from the research ethics board of Ardebil University of Medical Sciences (IR.ARUMS.REC.1403.227, 7 October 2024). The findings will be disseminated at local, national and international levels.
