In wild birds, the breeding season involves a convergence of metabolically demanding life-history stages, including reproduction, moult, and immune defense. We investigated the relationships between microfilarial infection, moult, redox homeostasis, and plumage quality in breeding male village weavers (Ploceus cucullatus) at the Amurum Forest Reserve, Nigeria. We compared four groups (n = 148 total) sampled within 3 mins post-capture: infected-moulting (IM), infected-non-moulting (IN), non-infected-moulting (NM), and non-infected-non-moulting (NN). Physiological condition was assessed using the erythrocyte glutathione ratio (GSH:GSSG) and circulating glucose, while plumage reflectance traits were integrated into a composite quality axis (PC1). Microfilarial infections were present in 52.0% (n = 77) of individuals; mean parasite intensities were 6.13 ± 0.35 mf/µL (IN) and 6.45 ± 0.41 mf/µL (IM). Physiological indices varied strongly across groups. The GSH:GSSG ratio was reduced in infected birds, indicating altered redox balance (rs = −0.65). Circulating glucose was highest in the infected non-moulting group (IN) but substantially reduced in the infected moulting group (IM). Across physiological and ornamental traits, individuals experiencing both infection and moult (IM group) exhibited the strongest reductions relative to all other groups. However, this pattern reflects a statistically supported Infection × Moult interaction, rather than an untested synergistic or non-linear effect, as evidenced by significant IN vs. IM contrasts in glucose (Table 3; z = 33.43, PThe growing advancement of innovative stem cell technologies requires careful evaluation of their economic, clinical and societal impacts. Early economic evaluations are essential for developing new medical technologies and supporting key decisions about commercialisation and market access. This scoping review explores Early Health Technology Assessment (eHTA) approaches specifically related to human stem cell technologies. By examining how eHTA can support the commercialisation of these therapies, we aim to clarify its role in optimising resource allocation and enhancing both the clinical and societal benefits of stem cell technologies.
To explore the use of eHTA in the development of stem cell-related technologies, a scoping review will follow the Preferred Reporting Items for Systematic Reviews and Meta-Analyses - Scoping Review Extension guidelines. Systematic searches were conducted across scientific databases (MEDLINE, International HTA database, EconLit, PAIS Index and EconPapers), grey literature sources (Overton) and through hand-searching to identify eligible articles published from inception to 14 April 2026. No limits were imposed on language. Reviewers will independently record data from eligible studies using a standard data abstraction form. The gathered information will be synthesised both quantitatively and narratively.
Formal ethical approval is not required, as this study does not involve the collection of primary data. The findings will be shared through professional stem cell networks, published in national and international health technology assessment conference proceedings and submitted for open-access, peer-reviewed publication.

BackgroundAccess to adequate sanitation remains a critical public health challenge in Somalia, where a large portion of the population relies on unimproved facilities due to persistent conflict, climate shocks, and political instability. This reliance contributes to a high burden of waterborne diseases. This study aimed to assess the spatial distribution of unimproved sanitation and identify its individual and community-level determinants using recent national data to inform targeted interventions.
MethodsThis study is a secondary analysis of the 2022 Somalia Integrated Household Budget Survey (SIHBS), which included 7,212 households. The primary outcome was the use of unimproved sanitation facilities, categorized according to the WHO/UNICEF Joint Monitoring Programme (JMP) definitions. We employed a multilevel logistic regression model to identify individual and community-level determinants associated with unimproved sanitation. To analyze the spatial patterns of unimproved sanitation, we used Global Moran’s I for spatial autocorrelation and the Getis-Ord Gi* statistic for hotspot analysis.
ResultsOverall, 36.87% of Somali households use unimproved sanitation facilities. There are significant disparities across residence types, with the highest prevalence among nomadic populations (83.28%), followed by rural (51.10%) and urban (23.88%) residents. The multilevel analysis revealed that households in permanent/formal housing (AOR: 3.42) and those with IDP status (AOR: 3.18) had significantly higher odds of using unimproved sanitation. At the community level, urban residence was paradoxically associated with higher odds of unimproved sanitation (AOR: 7.99) compared to rural areas, while nomadic populations had significantly lower odds (AOR: 0.04), likely reflecting a high prevalence of open defecation not captured as a “facility.” Spatial analysis identified significant hotspots of unimproved sanitation in the Hiraan (90.65%) and Bay (80.39%) regions, and cold spots in Banadir (5.37%) and Lower Shabelle (3.70%).
ConclusionThe findings highlight deep inequalities in sanitation access across Somalia, driven by geographic location, socioeconomic status, and population group. The high prevalence of unimproved sanitation, especially among nomadic, rural, and displaced populations, calls for urgent, geographically-targeted interventions. A multi-pronged approach is necessary, focusing on the specific needs of different communities and addressing the underlying structural and individual-level drivers of poor sanitation to advance public health and sustainable development goals in the region.
Sanaag and Sool are two remote regions in Somaliland with limited access to health service and high burden of HIV/AIDS. The aim of this study was to identify the predictors of HIV/AIDS awareness among women of reproductive age in underserved regions of Somaliland using data from the 2020 Somaliland Demographic and Health Survey through a cross-sectional analysis.

The study was conducted in the Sanaag and Sool regions of Somaliland. These areas are characterised by underserved communities and ongoing internal conflict, which has significantly undermined the delivery of health services. Consequently, there is a high HIV/AIDS burden in these regions. The study focused on women of reproductive age, using data from the 2020 Somaliland Demographic and Health Survey for the analysis.
The outcome variable of the study was awareness about HIV/AIDS and independent variables including education, media exposure, place of residence and wealth index were considered.
The proportion of poor HIV/AIDS awareness is high in Sool and Sanaag, with 38.8% and 26.6% of women respectively having no awareness about HIV/AIDS. Findings indicate that women who had primary and above level of education (adjusted OR, AOR=2.25; 95% CI 1.99 to 2.53) and media exposure including radio (AOR=2.31; 95% CI 1.99 to 2.68) and television (AOR=3.94; 95% CI 3.45 to 4.5) are strong predictors of HIV/AIDS awareness. Women in urban areas (AOR=2.83; 95% CI 2.25 to 3.57) were more likely to have HIV/AIDS awareness compared with women in rural and nomadic settings.
Inadequate awareness about HIV/AIDS was associated with education, residence and mass media exposure. Targeted health education programmes, promoting women’s education status and media campaigns could help improve HIV/AIDS awareness, which in turn enables reproductive age women to take protective measures against exposure.
Antenatal care (ANC) plays a critical role in improving maternal and neonatal health outcomes. However, incomplete ANC attendance in Somaliland is associated with adverse maternal and birth outcomes. Barriers to ANC attendance may increase the risk of pregnancy-related complications, including maternal morbidity, mortality and poor neonatal health outcomes. Understanding the effect of ANC attendance on maternal and birth outcomes is crucial for informing policies and interventions aimed at reducing these risks. Hence, this study aimed to assess the effect of ANC attendance on maternal and birth outcomes in Somaliland.
A prospective cohort study was conducted among 1205 pregnant women enrolled by systematic sampling method.
The study was conducted in the Republic of Somaliland, which is situated in the Horn of Africa. Baseline data were collected at recruitment, and participants were followed up to delivery for the collection of outcome variables. The number of ANCs was considered to be a dichotomous independent variable; incomplete attendance (≤ 3 visits) and complete attendance (≥4 visits). The risk of pregnancy outcome among those with incomplete ANC was assessed using multi-variable logistic regression.
The outcome variables of the study were the maternal and birth outcomes. The independent variables included socio-demographic characteristics, such as age, residence, educational status, occupation, family size, wealth index and marital status, and reproductive factors, such as parity, gestational age at first ANC visit, current pregnancy desirability and previous pregnancy history.
Out of the total participants, 43.3% of women had complete attendance. The incidence of postpartum haemorrhage was 10.0% (95% CI 8.6 to 12.3); antepartum haemorrhage, 3.6% (95% CI 2.6 to 4.7); caesarean section, 14.8% (95% CI 12.9 to 16.8); preterm delivery, 13.7% (95% CI 11.7 to 15.4); low birth weight, 25.8% (95% CI 23.4 to 28.1); and stillbirth, 3.2% (95% CI 2.3 to 4.2). Complete attendance to ANC significantly reduced the risk of antepartum haemorrhage, caesarean section, preterm delivery and admission to the neonatal intensive care unit and stillbirth.

This study aimed to address the spatial distribution and multilevel analysis of healthcare access barriers among women of reproductive age in Somalia.
The study was conducted across Somalia, an East African country facing significant spatial disparities in healthcare access. A cross-sectional study design was employed, using data from the 2020 Somali Demographic and Health Survey (SDHS). The data were analysed using both multilevel logistic regression and spatial analysis. To pinpoint barriers and identify statistically significant spatial clusters, the data were analysed using multilevel logistic regression in Stata V.17 and spatial analysis in R Studio (V.4.4.1), respectively.
The study population consisted of a weighted sample of 5118 women of reproductive age (15–49 years) from the SDHS.
Spatial analysis revealed significant regional heterogeneity, with high-prevalence areas concentrated in the northern region of Togdheer and a south-central cluster encompassing Galguduud, Hiiraan and Bakool. Multilevel analysis presented that women in the Bay region had nearly 10 times (AOR: 9.62) the risk of facing healthcare access barriers. While women in the highest quintile of wealth (AOR 0.21), those in higher education (AOR 0.30), those aged 45–49 (AOR 0.49) and not currently working (AOR 0.46) were significantly less likely to report access barriers.
Healthcare access barriers in Somalia are driven by a complex interplay of socioeconomic factors, specifically maternal age, education, employment and household wealth, and profound geographical disparities. Access barriers are not uniform but are geographically clustered in the south-central regions (Bay, Bakool, Hiiraan) and Togdheer in the northern region. Policy efforts must prioritise infrastructure investment in these identified high-burden hotspots while simultaneously dismantling systemic inequalities through the expansion of female education and financial protection schemes. This data-driven approach offers a definitive roadmap for decision-makers to equitably allocate resources and ensure that the most vulnerable populations are not left behind.
The timing of a woman’s first birth is significantly associated with maternal and child health outcomes and socioeconomic opportunities. Delaying the first birth is associated with reduced risks of adolescent pregnancy complications, improved birth outcomes and enhanced interpregnancy intervals, all of which contribute to better maternal and child health. In Somaliland, early childbearing is prevalent, necessitating an understanding of its associated factors. Early childbearing, defined as a first birth before age 20, is prevalent in Somaliland, with 47.02% of women having their first birth by this age, significantly higher than the average for sub-Saharan Africa (approximately 30%) and global figures.
This study aimed to identify the sociodemographic factors associated with age at first birth among married women in Somaliland.
The median duration from marriage to first birth was 3 years, with a median age at first birth of 21 years. Women residing in the Sool (adjusted HR: 1.318, 95% CI 1.138 to 1.527) and Sanaag (adjusted HR: 1.265, 95% CI 1.095 to 1.462) regions had a significantly higher hazard of first birth compared with those in Awdal (reference). Conversely, women in the middle (adjusted HR: 0.821, 95% CI 0.685 to 0.985) and highest (Adjusted HR: 0.799, 95% CI 0.663 to 0.964) wealth quintiles had a significantly lower hazard of first birth compared with the lowest wealth quintile. Age at first marriage was a strong factor associated with the outcome; marrying at age 20 or older (adjusted HR: 0.699, 95% CI 0.644 to 0.759) was associated with a significantly lower hazard of first birth compared with those who married before age 20. After adjustment, other factors including residence, media exposure and husband’s employment status were not significantly associated with the timing of first birth.
Region, wealth quintile and age at first marriage are critical factors associated with first birth timing in Somaliland. Interventions should focus on addressing regional disparities and promoting delayed marriage to improve maternal and child health outcomes. Further research is needed to explore the complex social and cultural drivers of these findings.
Patients with atrial fibrillation (AF) frequently have multiple comorbidities that increase the risk of hospitalisation and contribute to higher mortality. However, studies examining the prevalence of comorbidities among Middle Eastern patients with AF and their impact on clinical outcomes are scarce. This study aimed to assess the impact of comorbidities in a Middle Eastern population with AF treated with contemporary anticoagulation.
Prospective observational cohort study.
Patients from 20 hospitals and 30 outpatient cardiology clinics across Jordan were enrolled from May 2019 through October 2020.
2020 consecutive patients were enrolled. 117 of them were lost to follow-up, and 1903 had available data for analysis. Of the total, 1096 (54.3%) patients were women, and 924 (45.7%) were men. Eligible patients were 18 years of age or above, had a confirmed AF diagnosis and provided informed consent.
We are examining the outcomes of patients with AF, comparing those who have multimorbidities versus oligomorbidities. The primary outcomes were AF-related complications occurring within 1-year follow-up: major bleeding, non-major bleeding, stroke/cerebrovascular accidents, systemic emboli and acute coronary syndrome. Secondary outcomes included causes of death among deceased patients.
Among the cohort, 1160 (57.4%) had two or less comorbidities (oligomorbidity group) and 860 (42.6%) had three or more comorbidities (multimorbidity group). We propose the Fréchet–Power Function (FPF) distribution, a novel two-parameter model that combines the bounded support of the Power Function distribution with the heavy-tailed flexibility of a Fréchet-type generator. This combination enables the FPF to capture complex features such as skewness, heavy tails, and diverse hazard rate shapes—limitations present in existing bounded lifetime models. We provide explicit forms for the probability density, cumulative distribution, and quantile functions, along with detailed statistical properties including moments and hazard rate behavior. Parameters are estimated using maximum likelihood, with bootstrap and simulation techniques employed to assess estimator performance. Empirical applications to survival, reliability, and environmental datasets show that the FPF distribution consistently outperforms traditional models in terms of goodness-of-fit and flexibility. This work introduces a powerful and versatile tool for modeling bounded lifetime data, offering enhanced accuracy and interpretability across disciplines.Patient and family engagement in patient safety in low- and lower-middle-income countries (LLMICs) is not well described. This scoping review synthesised the literature to identify key strategies, barriers and enablers of engagement and to explain how patients and families contribute to safety efforts in healthcare settings.

PubMed, Medline, CINAHL, PsycINFO, Scopus and Web of Science were searched from inception to April 2024.
We included qualitative, quantitative and mixed-methods studies, published in English across all healthcare settings, that met the following criteria: (1) explored the attitudes, perceptions and experiences of patients, families or healthcare providers (HCPs) regarding patient engagement in safety activities; (2) described the participation of patients, caregivers or families in the design, delivery or evaluation of interventions aimed at promoting patient safety in healthcare facilities.
Data were extracted using a standardised charting form, and a narrative synthe.sis was conducted to identify key themes. The National Health Service (NHS) framework for patient engagement in patient safety was applied to categorise forms of engagement (information, involvement and partnership) and levels of engagement (own care, service provider and system) among included studies.
Of 7709 records screened, 36 studies met inclusion criteria. Twenty-three studies explored the perspectives of patients, families and HCPs on patient engagement in safety. Key findings revealed that while patients and families recognised the importance of their roles in safety, unclear role definitions, lack of training and communication barriers limited their participation. HCPs acknowledged the value of patient involvement but identified challenges such as knowledge gaps, time constraints and lack of institutional support. Thirteen studies involved patients and family caregivers in safety improvement initiatives. Most interventions were limited to information sharing, with only two achieving partnership-level engagement. Engagement primarily occurred at the individual and provider levels, with no studies addressing the policy level. Factors influencing engagement were identified at four levels: patient, HCP, system and community.
Despite its potential benefits, the implementation and impact of patient and family engagement in patient safety efforts in LLMICs remain largely unexplored. Most efforts focus on information sharing through educational interventions at the direct care level, with limited involvement of patients and families at the partnership level. To enhance safety outcomes, future initiatives must adopt context-specific strategies that empower patients and families as active partners. Drawing lessons from high-income countries and adapting them to the socio-cultural and infrastructural realities of LLMICs will be crucial for developing scalable, cost-effective and sustainable engagement practices.
Implementation science research increases the uptake of evidence-based interventions, which may improve health equity among racial and ethnic minorities. However, it is unclear how anti-racism and anti-colonialism practices have been integrated into implementation science research. The objectives of this scoping review are to describe the current conceptualisations of racism and colonialism within the USA, examine racism or colonialism-conscious approaches and analyse gaps in the operationalisation of anti-racism or anti-colonialism within implementation science studies.
This scoping review will be conducted following the Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for scoping reviews guidelines. The Center for Chronic Disease Reduction and Equity Promotion Across Minnesota conceptual framework and an implementation science anti-racism lens will guide the study design and analysis. To determine study eligibility for the scoping review, articles will undergo abstract and full-text screening by two independent reviewers and discrepancies will be settled together. Data charting will be extracted from included articles by eight independent reviewers. The search strategy will use controlled vocabulary and natural language keywords related to health equity, health disparities and anti-racism/colonialism on six databases. The scoping review will include studies that applied implementation science theories, models or frameworks among US-based populations. Additionally, included studies will report any of the following implementation activities: implementation strategies, implementation outcomes, adaptations to evidence-based interventions, or evaluations of pre-implementation or implementation context.
No ethical approval was required for the scoping review. Dissemination will be through publication in a peer-reviewed journal and conference presentations.

This study shows the important determinant factors associated with SAM relapse among 6–59-month-old children after discharge from outpatient therapeutic programmes in Godey City and district in Eastern Ethiopia. The significantly associated factors, including household food insecurity, inadequate hand-washing practices, shorter treatment duration, low MUAC at discharge and lack of vitamin A supplementation, indicate the multifaceted nature of this public health problem.
Enhancing household food security through community-based programmes, promoting health education on proper hand washing and hygiene practices, extension of outpatient treatment duration to prevent premature discharge, ensuring that MUAC threshold for discharge is properly attained, and integrating postdischarge Vitamin A supplementation into primary healthcare services.
The study aims to assess the magnitude of acute gastroenteritis and associated factors among under-five children visiting public hospitals in Jigjiga City, Somali Region, Ethiopia.
A hospital-based cross-sectional study design was used to carry out the study. We then employed a systematic random sampling technique through face-to-face interviews to gather the data. A structured questionnaire consisting of socio-demographic, behavioural, environmental and clinical factors was developed after reviewing relevant literature.

The primary outcome of the study was the prevalence of acute gastroenteritis.
The 2-week prevalence of acute gastroenteritis among under-five children was 24.6% (95% CI 23.4% to 25.9%). The study found strong links between acute gastroenteritis and having a household greater than five (adjusted OR (AOR)=2.94, 95% CI 1.55 to 5.60), an unprotected source of drinking water (AOR=2.74, 95% CI 1.29 to 5.82), unimproved latrine facilities (AOR=3.15, 95% CI 1.26 to 7.78), the presence of faeces around the latrine (AOR=3.53, 95% CI 1.77 to 7.03) and mothers or caregivers’ history of diarrhoea over the past 2 weeks (AOR=6.23, 95% CI 3.22 to 12.06).
The overall prevalence of acute gastroenteritis in Jigjiga City was relatively high in the 2-week period. Having a household greater than five, an unprotected source of drinking water and unimproved latrine facilities was a significant predictor of acute gastroenteritis. Additionally, the presence of faeces around the latrines and the history of diarrhoea in mothers or caregivers over the past 2 weeks were strongly linked to acute gastroenteritis.
The goal of the study was to determine the magnitude and contributing factors of low back pain among primary school teachers in Borama Town, Somaliland.
An institution-based descriptive cross-sectional study design was employed. Simple random sampling was used to select the study units from each school.

The study found that 51.5% of school teachers had low back pain. There was a strong link between low back pain and having a higher Body Mass Index (adjusted OR (AOR)=2.63) and stress at work (AOR=3.34). Sleep disturbance (AOR=1.73), lifting heavy materials (AOR=1.67) and a history of low back injury (AOR=2.12) were also significant predictors of low back pain.
More than half of primary school teachers had low back pain over the past 12 months. Higher Body Mass Index, history of low back injury, stress at work, lifting heavy material and sleep disturbance were significant and independent predictors of low back pain among primary school teachers.

The study included 341 hypertensive individuals, with a 99.1% response rate. Of the respondents, 58.9% were men. The overall prevalence of CVD risk within the coming 10 years was 134 (39.3%; 95% CI: 34.1% to 44.5%). Multivariable logistic regression analysis identified age, khat chewing, smoking and comorbid conditions as significant independent predictors of CVD risk. Specifically, individuals aged 60–69 years had an adjusted OR (AOR) of 3.97 (95% CI: 1.94 to 8.16) and those aged 70–74 years had an AOR of 2.99 (95% CI: 1.57 to 5.71). Khat chewers had an AOR of 2.58 (95% CI: 1.22 to 5.46), smokers an AOR of 3.44 (95% CI: 1.59 to 7.48) and individuals with comorbidities an AOR of 2.42 (95% CI: 1.47 to 3.99).
There is a significant increase in 10-year CVD risk among hypertensive patients in the study area. Age, khat chewing, smoking and comorbidities were independent predictors. Regular CVD risk screening for older patients, focused health education to reduce khat and tobacco use and integrated management of comorbidities are essential to lower future cardiovascular risk.
