Beyond Training: A Qualitative Meta‐Synthesis of Healthcare Professionals' Experiences Providing Culturally Competent Antepartum and Intrapartum Care to Ethnic Minoritized Women

ABSTRACT

Background

Ethnic minoritized women face cultural and systemic barriers in accessing antepartum and intrapartum care. Healthcare providers play a pivotal role in addressing these challenges, but their perspectives and experiences in delivering culturally competent care remain underexplored.

Aim

To synthesise healthcare providers' experiences and perspectives on providing culturally competent antepartum and intrapartum care for ethnic minoritised women.

Methods

A qualitative meta-synthesis study design was employed. Six electronic databases were searched from their inception date till January 2025. The included studies were assessed using the method of the Critical Appraisal Skills Programme tool, and findings were meta-synthesised using Sandelowski and Barroso's six-step approach. This review was registered via the International Prospective Register of Systematic Reviews.

Results

Overall, 38 studies were included, and three themes emerged. The first theme revealed how providers' biases and professional training distorted their ability to understand and respect cultural practices. The second theme underscored the impact of systemic barriers such as time constraints, resource scarcity and lack of representation among providers. The final theme highlighted healthcare providers' aspirations for improved communication, targeted training and guidance on building trust to enhance care delivery.

Conclusion

Healthcare providers encounter notable challenges in delivering culturally competent antepartum and intrapartum care, but remain hopeful about bridging gaps in communication and understanding. Practical recommendations include implementing mandatory cultural competency training at all levels of healthcare professional education, increasing resources for interpreters and cultural liaisons and fostering diversity within the healthcare workforce. Future research should explore patient-centred interventions and systemic reforms to improve care for ethnic minoritised women. These findings highlight the need for policies and practices that empower providers to deliver equitable, culturally respectful antepartum and intrapartum care.

Perceptions of Nurses, Patients, and Family Members on Rooming‐In in Adult Care: A Cross Sectional Survey Study

ABSTRACT

Aim

To better develop and understand the practice of rooming-in in adult care, this study aims to explore the perceptions of the three most important stakeholders: nurses, patients, and family members.

Design

A cross sectional survey study in a university medical centre in the Netherlands.

Methods

A convenience sample was drawn from nurses, adult patients, and family members across ten general wards. Data of patients and family members were collected through online questionnaires, developed and validated prior to the study. Their responses were summarised using descriptive statistics. For the nurses, data were obtained by analysing responses to an open-ended question from an existing questionnaire, using thematic analysis.

Results

The sample consisted of 364 nurses, 26 patients, and 35 family members. Thematic analysis of the nursing data revealed four themes: (1) reducing the workload of nurses, (2) optimising patient recovery, (3) unclear policy and inadequate facilities, and (4) complexities for nurses. Regarding the perceptions of patients and family members, more than 75% perceived that rooming-in enhanced patients' sense of safety. Additionally, 54.1% of family members indicated that rooming-in improved their understanding of medical information. For most patients (57.7%) and family members (62.9%), the rules and conditions for rooming-in were not clear. Moreover, 65.7% of family members reported experiencing physical burden. Both patients and family members rated rooming-in with a median of 8 out of 10.

Conclusion

According to nurses, patients, and family members, rooming-in may be beneficial in reducing nurses' workload, enhancing patient safety, and improving family understanding of medical information.

Implications for Clinical Practice

Given the ambiguity surrounding rooming-in policies in adult care, clear guidelines and their implementation are essential for ensuring their success.

Impact

Rooming-in can positively contribute to adult care by facilitating family involvement.

Understanding Nurses' Needs Regarding Tailored, Evidence‐Based Sleep Education and Training

ABSTRACT

Aim

Identify desired training content for shift-working nurses to improve their sleep and fatigue.

Design

A descriptive qualitative design.

Methods

We recruited night shift nurses (N = 23) to provide feedback during virtual focus groups/interviews. Data collection occurred in the U.S. between March and June 2024. Participants were presented with sleep and fatigue topics derived from the literature. Focus group/interview data were collected and transcribed. Data were analysed using a hybrid deductive-inductive manifest content analysis with an a priori coding schema based on topics shared during data collection. Data not fitting the schema, yet informing content, were analysed inductively.

Results

Three themes aligned with literature-derived topics. Theme 1, Why We Sleep and Why Should Nurses Care, explains the importance of sleep to health. Theme 2, Sleep Practices for Nurses to Support Health and Social Relationships, describes healthy strategies to promote sleep for enhanced quality of life. Theme 3, Fatigue and Work, illustrates the significance of nurse sleep and fatigue risk mitigation to safe working conditions and patient care.

Conclusions

Study findings highlight night shift nurses' interest in gaining evidence-based information to promote their sleep. Sleep education and training could fill a knowledge and skills gap, not often offered in school or workplace.

Implications for the Profession

Identifying themes relevant to nurses may help increase the development and availability of sleep education and training currently tailored for nurses.

Impact

Study findings describe content night shift nurses' desire for sleep and fatigue training, serving as an important first step in developing programmes most relevant to shift-working nurses. Our analysis found the findings largely align with key components workers should receive in sleep education and training and reinforced the need for employers to offer such training. This study could benefit the nursing workforce and employers who expect rested, high-functioning nurses to care for patients.

Advanced Practice Nurses' Evidence‐Based Healthcare Competence and Associated Factors in Finland and Singapore—A Cross‐Sectional Study

ABSTRACT

Aim

To describe and compare the Evidence-Based HealthCare (EBHC) competence of Advanced Practice Nurses (APNs), and the factors associated with it in Finland and Singapore.

Design

A descriptive and analytical cross-sectional study.

Methods

Data were collected from APNs working in healthcare in Finland (n = 157) or Singapore (n = 99) between May 2023 and October 2023 using a self-assessment instrument to measure EBHC competence (EBHC-Comp-APN) and an EBHC knowledge test. The data were analysed using descriptive statistics, analysis of variance, K-mean cluster and multivariate analyses.

Results

The self-assessments of APNs working in Finland and Singapore regarding their EBHC competence level varied and three distinct profiles of APNs' EBHC competence were identified in both countries. The strongest EBHC competence was in ‘The Knowledge Needs Related to Global Health’, while the weakest in ‘Evidence Synthesis and Transfer’. The country-specific differences were identified in factors associated with EBHC competence.

Conclusion

The EBHC competencies of APNs vary widely and require planned and needs-driven development. In connection with the development of EBHC competence, the factors related to competence should be considered country-by-country.

Implications for the Profession

The APN's EBHC competence should be systematically developed considering the factors associated with and the current level of EBHC competence.

Impact

The level of EBHC competence of APNs and associated factors should be identified when developing their competence and role in collaboration with APNs, leaders of healthcare and education organisations and policy makers. In addition, research into APNs' EBHC competence should continue.

Recommendations for Mental Health Chatbot Conversations: An Integrative Review

ABSTRACT

Aim

To identify and synthesise recommendations and guidelines for mental health chatbot conversational design.

Design

Integrative review.

Methods

Suitable publications presenting recommendations or guidelines for mental health conversational design were included. The quality of included publications was assessed using Joanna Briggs Institute Critical Appraisal Tools. Thematic analysis was conducted.

Data sources

Primary searches limited to last 10 years were conducted in PubMed, Scopus, ACM Digital Library and EBSCO databases including APA PsycINFO, CINAHL, APA PsycArticles and MEDLINE in February 2023 and updated in October 2023. A secondary search was conducted in Google Scholar in May 2023.

Results

Of 1684 articles screened, 16 publications were selected. Three overarching themes were developed: (1) explicit knowledge about chatbot design and domain, (2) knowing your audience and (3) creating a safe space to engage. Results highlight that creating pleasant and effective conversations with a mental health chatbot requires careful and professional planning in advance, defining the target group and working together with it to address its needs and preferences. It is essential to emphasise the pleasant user experience and safety from both technical and psychological perspectives.

Conclusion

Recommendations for mental health chatbot conversational design have evolved and become more specific in recent years. Recommendations set high standards for mental health chatbots. To meet that, co-design, explicit knowledge of the user needs, domain and conversational design is needed.

Implications for the Profession and/or Patient Care

Mental health professionals participating in chatbot development can utilise this review. The results can also inform technical development teams not involving healthcare professionals directly.

Impact

Knowledge of developing mental health chatbot conversations appears scattered. In mental health chatbots, features that enhance the chatbot's ability to meet users' needs and increase safety should be considered. This review is useful for developers of mental health chatbots and other health applications used independently.
